For Patients

New patients will need to fill out and fax or email back the new patient registration and information forms before the first appointment. We request 48 hours’ notice for appointment cancellations, otherwise a cancellation fee maybe charged.

Consultation Requirements

All patient information including Referral, TED Clinics Patient registration form and Medical Questionnaire must be emailed or faxed to TED Clinics prior to the appointment.

What to bring to your appointment:

  • Any relevant test results including blood tests, radiology reports, and any relevant correspondence from other specialists.
  • All current medications
  • A list of names and addresses of all doctors currently involved in your care
  • For people with diabetes – a record of your blood glucose readings
  • If you have been referred for Transport for NSW medical assessment for Fitness to Drive bring diabetes blood and urine screening test results within the previous 3 months from your GP, results of a recent ophthalmologist or optometrist review for retinopathy, and record of blood glucose levels from the preceding month and blood glucose monitor used.

For new patients the appointment duration allotted is 30 minutes to 45 minutes.  For follow-up appointments, time allotted is 15 minutes to 20 minutes.

Fee Structure
Fees are determined by individual practitioners. An out-of-pocket cost will be incurred for all consultations.
Payment by EFTPOS, credit card or cash are accepted, and is expected on the day of consultation.
